B) Download Retail Customer Order Packet containing all information to order the Easy Gait Brace.

 

C) Include Order Form, Measurement Sheet and Reimbursement Forms (if applicable) 

with your shoe/boot/sneaker in a U.S. Postal Service Priority Mail Envelope to:
